Program Overview:
As part of the Nephrology Program, this role supports chronic disease prevention and management using a patient-centered and self-management approach aligned with the Ontario Renal Network and Ontario Renal Plan 4.
Position Summary:
The Registered Nurse – Support Nurse works within the Home Hemodialysis Program, supporting patients in Long-Term Care Satellites and the Transitional Care Unit. The RN is responsible for delivering comprehensive nursing care to a specialized nephrology patient population.
Hours & Shift:
Site operates 7 days per week
Must be available for Day and Evening shifts
8-hour and 12-hour shifts
On-call duties as assigned
Key Responsibilities:
Provide comprehensive care for patients receiving hemodialysis
Educate and follow up with hemodialysis patients
Deliver primary nursing care and coordinate plans of care
Collaborate with multidisciplinary healthcare teams
Perform patient assessments and emergency interventions
Required Qualifications:
Current RN registration with the College of Nurses of Ontario (CNO)
BCLS certification upon hire
Minimum 1 year acute care experience
Nephrology certification or commitment to complete
Basic computer skills; Meditech experience an asset
